PIEDMONT HEALTH CARE CENTER 12-BED REHAB ADDITION

The Piedmont Health Care Center completed in April of 2021, a new 6750 SF, 12-bed, all private room addition to their existing 85,000 SF facility. This addition increased our private room capacity to 45 total private resident rooms available in our skilled nursing facility. 

The person-centered care model includes all private rooms with private baths and open therapy space for a home-like experience.  The home-like scale of the addition offers a 12-bed household, designed to maximize staff efficiency while providing a smaller-scale experience in an environment created for residents to socialize and perform regular daily activities together. 

The building exceeds the strict health and safety standards of the State of Alabama Department of Public Health while meeting the design and programming objectives established by the owners and leadership team.

Your physician will provide all the initial instructions for your rehabilitation care and then we meet to set your specific transitional physical therapy care goals and establish your individual rehabilitation care plan. Once the plan is established, we both get to work. You will have a far better chance to recover if you are fully informed of your rehabilitation care options and sign off on your own recovery plan.

When the time comes to go home, we help you arrange a gentle step back home. We will help you organize the support services needed such as home care, equipment, outpatient or home therapy, technical safety options, and physician follow-up to assure a safe and successful transition back home.

The facility is dually licensed by the State of Alabama and Centers for Medicare and Medicaid (CMS) to provide both long-term care services and short-term rehabilitative services.

Currently licensed for 91 nursing home beds by the Alabama Department of Public Health, our facility is certified for Medicare and Medicaid admissions as well as private insurance coverage. 

At PHCC, we are pleased to provide excellence in long-term nursing care for our residents who need medical/rehabilitative and restorative care. Our comprehensive plan of care for each resident is physician-driven and includes an individualized assessment with well-trained clinicians as well as the resident. This assessment will determine the appropriate level of care required, including administration of medications and treatments, restorative and rehabilitative care, nutritional requirements, psychosocial needs and activities of daily living.

Piedmont Health and Rehabilitation Center achieved a Five-Star rating as measured by CMS (see Medicare.gov). Sandra Keener is the Administrator for the facility and Dr. Benjamin Ingram is the Chairman of the Board of Directors.
